Registering and Signing Climb Youngstown Waiver
INSTRUCTIONS
Visit the Climb Youngstown website (HERE) and Click “Sign a Waiver”
Create an account WITH PARENT OR GUARDIAN’S email.
Fill out your personal information (parent or guardian) and finalize your account creation.
Once in your account, click “Household” in the menu on the left and add the student(s) attending our programs and save them under your profile.
Click “Sign A Waiver,” in the menu on the left, select their name (or select all) and proceed to fill out the waiver.
Following proper signatures and initialing of the waiver, submit the form.
